Housing Pathways to Better Health: Learning from elsewhere

Join our Health and Housing Impact Network online disucssion to learn about successful work from around the country, involving the NHS, local authority and community partners. Continuing our focus on integrated care, you will hear about ways to:

  • mainstream housing, health and care integration, strategically and practically;
  • foster a multidisciplinary culture which underpins a “pathway” from diagnosis or identification of a problem to treatment or response and ongoing care;
  • improve people’s living conditions, prevent ill health and reduce inequalities.

Through interactive discussion, we'll see how we can apply learning to London from these fantastic examples from across England.
